Поскольку apache poiXWPF на самом деле в бета-состоянии до сих пор такие вещи возможно только, если один точно знает, как Word будет хранить это содержимое в XML. Тогда можно обойти недостатки apache poiXWPF. Вы уже использовали такое обходное решение, которое исправляет пропущенный blipID, когда изображения добавляются в пробелы в верхнем или нижнем колонтитуле.

Чтобы узнать, как Word сохранит содержимое в XML не является наукой о ракетном оружии. Файл *.docx - это просто файл ZIP. Если один раз разархивировать этот файл с помощью программного обеспечения Zip, можно просто просмотреть XML-файлы.

Насколько я знаю, добавление форм (в данном случае текстовых полей) в документы Word не поддерживается напрямую apache poi. Для этого используются низкоуровневые объекты (в данном случае CTGroup и CTShape).
