Форма Php с переключателем не работает

Question

У меня есть html-форма с php. все работает нормально, кроме переключателя. когда я получаю электронную почту, Контактная информация пуста.

У меня есть некоторая ошибка в файле php. Pls помочь мне

Я думаю, что вам не хватает $contact = $_POST['contact'];!
