Обратите внимание:

• Я пробовал mysqli безрезультатно ... не знаю, почему, но он никогда не работает для меня.
• Я прочитал несколько дублированных сообщений ..... все еще не уверен, что не так.

Отсутствие сообщения об ошибке или успехе. В столбце Имя есть значение NULL, вставленное в него.
